Lens-based technologies to study accommodation and refraction
A visual stimulus can be presented in free space or through lens-based systems. Interestingly, it has been reported many times in the past that some subjects have poorer accommodative responses when optically stimulated, with no aparent reason. However, this limitation has not precluded the use of lens-based systems in ophthalmic devices, virtual reality displays or research setups, probably because it is outweighed by some advantages such as the computer-control of the focal plane of the stimuli. Under this general context, the objective of this thesis is to apply lens-based technologies (from computer-controlled electró-optical lenses to Badal Systems) to study accommodation and refraction.
The methodology of this thesis is structured in 6 studies. The first three studies investigate the response of the accommodative system when optically-stimulated with a Badal optometer. The remaining 3 studies take advantage of electro-optical varifocal systems to investigate new methodologies related to the automated subjective refraction and the accommodative facility test. Concretely, the fourth study is a clinical validation of a new automated refraction algorithm and is the only study thatworks specifically on eye¿s refraction. The fifth and sixth studies use an electro-optical liquid lens to present a repeated step-like stimulus at different accommodative distances. The fifth study validates a new accommodative facility test that integrates both the far and near accommodative facility testwith random changes of accommodative stimulus and, the sixth study explores how the predictability of a stimulus affects the accommodation dynamics.
The results of the first three studies show that previously reported differences in accommodative response when using lens-based methods compared to free space viewing may be explained by the effect of other factors such as the refractive error or field of view rather than the method to stimulate accommodation. It is also shown that the most accurate accommodative responses are obtained for fields between 8º and 10º which suggests that there may be an optimum peripheral retinal image size for accommodation stimulation. Also, it is shown that the only factor that in isolation significantly affects the accuracy of the accommodative responses is the type of refractive error. And finally, it is also shown that the accuracy of the accommodation response generally improves with a 2-dimensional stimulus with apparent depth cues and simulated out-of-focus blur in a relatively large field of view. Even though these conditions may not be adequate for all individuals, they can be used to improve the overall visual comfort in those virtual reality systems that use a varifocal optical system to change the focal plane of a 2-dimensional surface or visual simulators.
The results of the study 4 show the first implementation of a potential novel method of performing non1cycloplegic subjective refraction in adults without clinician suport. Although this method has some limitations thatwarrant further research and it should be tested in a wider population in terms of age4 refraction and different ocular conditions4 it is precise and more accurate than objective refraction methods and it has the potential to be incorporated in novel lens1based technologies to improve primary eye care services in developing countries.
And finally, the last two studies showed also the first implementation of a new accommodative facility test that integrates both the far and near accommodative facility testwith random changes of accommodative stimulus. This new methodology is a faster test than performing both the near and far accommodative tests and it provides more information than conventional accommodative facility tests. Additionally these two studies showed that the prediction operator does not exist in accommodation and the unpredictability of the stimulus does not affect the accommodation dynamics.Un estÃmul visual es pot presentar a l'espai lliure o a través de sistemes basats en lents. Curiosament, en el passat s'ha reportat moltes vegades que alguns subjectes acomoden de forma més pobre quan aquesta és estimulada per mitjans òptics. Tanmateix, aquesta limitació no ha impedit l'ús de sistemes basats en lents en dispositius oftà lmics, sistemes de realitat virtual o muntatges òptics de recerca, probablement perquè es compensen les limitacions amb alguns avantatges com el control per ordinador del pla focal del estÃmul. En aquest context general, l'objectiu d'aquesta tesi és aplicar tecnologies basades en lents (des de lents electró-òptiques controlades per ordinador fins a sistemes Badal) per estudiar l'acomodació i refracció. La metodologia d'aquesta tesi està estructurada en 6 estudis. Els primers tres estudis investiguen la resposta del sistema acomodatiu quan està estimulat òpticament amb un optòmetre Badal. Els 3 estudis restants aprofiten els sistemes varifocals electró-òptics per investigar noves metodologies relacionades amb la refracció subjectiva automà tica i la prova de flexibilitat acomodativa. Concretament, el quart estudi és una validació clÃnica d'un nou algorisme de refracció automatitzat i és l'únic estudi que treballa especÃficament en la refracció ocular. Els estudis cinquè i sisè utilitzen una lent òptica electró-òptica per presentar un estÃmul repetit a diferents distà ncies acomodatives. El cinquè estudi valida una nova prova hÃbrida acomodativa que integra la prova de flexibilitat acomodativa llunyana i propera amb canvis aleatoris d'estÃmul acomodatiu i, el sisè estudi explora com la predictibilitat d'un estÃmul afecta la dinà mica acomodativa. Els resultats dels tres primers estudis mostren que les diferències reportades anteriorment en la resposta acomodativa quan s'utilitzen mètodes basats en lents en comparació amb la visualització d'estÃmuls en l'espai lliure es pot explicar per l'efecte de factors com l'error refractiu o el camp de visió més que en el mètode d'estimulació d'acomodació. També es demostra que l'acomodació és més precisa per a camps visuals entre 8 i 10º, el que suggereix que hi pot haver una mida d'imatge retiniana òptima per a l'estimulació de l'acomodació. A més, es demostra que l'únic factor que afecta aïlladament l'exactitud de la resposta acomodativa és el tipus d'error refractiu. També, es demostra que la precisió de la resposta acomodativa generalment millora amb un estÃmul bidimensional amb pistes de profunditat perifèriques i desenfoc simulat per a un camp de visió relativament gran. Aquestes condicions es poden utilitzar per millorar el confort visual en aquells sistemes de realitat virtual o simuladors visuals que utilitzen un sistema òptic varifocal per canviar el pla focal d'una superfÃcie bidimensional. Els resultats de l'estudi 4 mostren la primera implementació d'un nou mètode potencial per a la realització de refracció subjectiva no cicloplègica en adults sense suport clÃnic. Encara que aquest mètode requereix encara una investigació addicional en una població més à mplia en termes d'edat, refracció i condicions oculars diferents, és més precÃs que els mètodes de refracció objectius i té el potencial d'incorporar-se a noves tecnologies basades en lents per millorar els serveis d'atenció primà ria en països en vies de desenvolupament. Finalment, els dos últims estudis mostren la primera implementació d'una nova prova de flexibilitat acomodativa que integra tant la prova llunyana i propera, amb canvis aleatoris d'estÃmul acomodatiu. Aquesta nova metodologia és una prova més rà pida que l'execució de les proves d'acomodació pròxima i llunyana i proporciona més informació que les proves de flexibilitat acomodativa per separat. Addicionalment, aquests dos estudis mostren que la capacitat predictiva en l'acomodació es insignificant i que la imprevisibilitat de l'estÃmul no afecta la dinà mica acomodativ
Repetibilitat de dos aberròmetres comercials
Fa molts anys que és coneguda la importà ncia que juguen les aberracions òptiques en
la visió de les persones, és per això que ja fa temps que s’intenten mesurar amb
precisió. Aquest fet sempre ha suposat tot un repte pels cientÃfics donada la dificultat
d’accés a l’espai imatge de l’ull (la retina). No obstant, grà cies als avenços tecnològics
dels últims anys s’han aconseguit desenvolupar noves tècniques i nous instruments
que han permès que la mesura d’aberracions pugui efectuar-se en la prà ctica clÃnica.
Entre les tècniques més utilitzades hi ha la de Hartmann-Shack.
En aquest treball s’ha avaluat la repetibilitat intrasessió i intersessió de dos
aberròmetres basats en la tècnica de Hartmann-Shack: el KR-1W (Topcon Corp.,
Tokyo, Japan) i l’AOVA (Voptica, S.L., Murcia, Spain). Per fer aquesta avaluació s’han
realitzat mesures en una mostra de població sana de 31 subjectes i s’han utilitzat onze
parà metres per a la seva quantificació.
Els onze parà metres analitzats són: Coma eix Y Z(3,-1), Coma eix X Z(3,1), Trefoil
Z(3,-3), Trefoil Z(3,3), Aberració esfèrica Z(4,0), Coma secundari Z(5,-1), Coma
secundari Z(5,1), refracció objectiva (Esfera i Cilindre), RMSh (alt ordre) i RMSt (total).
Per l’anà lisi estadÃstic de repetibilitat intrasessió i intersessió s’ha utilitzat el mètode
proposat per Bland i Altman. A més, per al cas d’intrasessió s’han calculat els
coeficients de repetibilitat (CORs). Per valorar la normalitat dels resultats s’ha utilitzat
el test Kolmogorov-Smirnov. Si era possible un anà lisi paramètric s’ha utilitzat
l’ANOVA, sinó el test Wilcoxon. També s’ha calculat el coeficient de correlació
intraclasse (ICC) aplicat al conjunt de les 9 mesures.
El resultats suggereixen que no hi ha diferències estadÃsticament significatives pel que
fa a la repetibilitat intrasessió i intersessió en el dos aparells. Pel KR-1W, els CORs
dels coeficients de Zernike estan compresos entre 0.01 i 0.05, els CORs de l’esfera i
cilindre entre el 0.09 i 0.25, i els CORs de les RMS entre 0,02 i 012. Per l’AOVA, els
CORs respectius estan compresos entre 0.05 i 0.18, 0.29 i 0.37, 0.16 i 0.21. El ICC
promig obtingut pel KR-1W és de 0.957 i el ICC promig per l’AOVA de 0.811

Effect of Experimental Conditions in the Accommodation Response in Myopia
SIGNIFICANCE
The accommodative response is more affected by the type of refractive error than the method of stimulation, field of view (FOV), or stimulus depth.
PURPOSE
This study aims to analyze the effect of stimulation method, stimulus depth, and FOV on the accommodation response (AR) for emmetropes (EMM), late-onset myopes (LOM), and early-onset myopes (EOM).
METHODS
Monocular AR was measured in 26 young observers (n = 9 EMM, n = 8 LOM, n = 9 EOM) under 60 different viewing conditions that were the result of permuting the following factors: (1) stimulation method (free space or Badal lens viewing), (2) stimulus depth (flat or volumetric), (3) FOV (2.5, 4, 8, 10, and 30°), and (4) accommodative stimulus (AS: 0.17, 2.50, and 5.00 diopters [D]).
RESULTS
Mixed analysis of variance for 2.50 D of AS resulted in a significant effect of refractive group (F = 6.77, P < .01) and FOV (F = 1.26, P = .04). There was also a significant interaction between stimulus depth and FOV (F = 2.73, P = .03) and among stimulation method, FOV, and refractive group (F = 2.42, P = .02). For AS of 5.00 D, there was a significant effect of refractive group (F = 13.88, P < .01) and stimulation method (F = 5.16, P = .03). There was also a significant interaction of stimulation method, stimulus depth, and refractive group (F = 4.08, P = .03). When controlling for all interactions, LOM showed larger lags than EMM and EOM; the AR did not significantly change for fields of 8, 10, and 30°, and it did not significantly differ for different stimulation methods or stimulus depth.
CONCLUSIONS
Previously reported differences in AR when using lens-based methods compared with free space viewing may be explained by the effect of other factors such as the FOV or the depth of the stimulus. Targets with an FOV of 8 or 10° may be optimal for accurate ARs.Preprin

Effect of apparent depth cues on accommodation in a Badal optometer
Background:
The aim was to analyse the effect of peripheral depth cues on accommodation in Badal optometers.
Methods:
Monocular refractions at 0.17 and 5.00 D of accommodative stimulus were measured with the PowerRef II autorefractor (Plusoptix Inc., Atlanta, Georgia, USA). Subjects looked (randomly) at four different scenes, one real scene comprising familiar objects at different depth planes (Real) and three virtual scenes comprising different two-dimensional pictures seen through a Badal lens. The first image consisted of a photograph of the real scene taken in conditions that closely mimic a healthy standard human eye performance (out-of-focus [OoF] blur); the second image was the same photograph rendered with a depth of focus to infinity (OoF sharpness); and finally the third image consisted of a fixation target and a even white surrounding (White). In all cases the field of view was 25.0° and the fixation target was a Maltese cross subtending to two degrees.
Results:
Twenty-eight right eyes from healthy young subjects were measured. The achieved statistical power was 0.9. At 5.00 D of accommodative stimulus, the repeated measures analysis of variance was statistically significant (p¿<¿0.05) and the corresponding Bonferroni post hoc tests showed the following mean accommodative response differences and standard deviation (p-value) between the real and the virtual scenes: real-white¿=-0.66¿±¿0.92 D (p¿<¿0.01); real-OoF sharpness¿=¿-0.43¿±¿0.88 D (p¿=¿0.07); real-OoF blur¿=-0.25¿±¿0.93 D (p¿=¿0.89).
Conclusions:
A stimulus poor in depth cues inaccurately stimulates accommodation in Badal optometers; however, accommodation can be significantly improved in the same Badal optometer, when displaying a realistic image rich in peripheral depth cues, even though these peripheral cues (also referred to as retinal blur cues) are shown in the same plane as the fixation target. These results have important implications in stereoscopic virtual reality systems that fail to represent appropriately retinal blur.Preprin
Suitability of open-field autorefractors as pupillometers and instrument design effects
AIM: To determine the agreement and repeatability of the pupil measurement obtained with VIP-200 (Neuroptics), PowerRef II (Plusoptix), WAM-5500 (Grand Seiko) and study the effects of instrument design on pupillometry.
METHODS: Forty patients were measured twice in low, mid and high mesopic. Repeatability was analyzed with the within-subject standard deviation (Sw) and paired t-tests. Agreement was studied with Bland-Altman plots and repeated measures ANOVA. Instrument design analysis consisted on measuring pupil size with PowerRef II simulating monocular and binocular conditions as well as with proximity cues and without proximity cues.
RESULTS: The mean difference (standard deviation) between test-retest for low, mid and high mesopic conditions were, respectively: -0.09 (+/- 0.16), -0.05 (+/- 0.18) and -0.08 (+/- 0.23) mm for Neuroptics, -0.05 (+/- 0.17), -0.12 (+/- 0.23) and -0.17 (+/- 0.34) mm for WAM-5500, -0.04 (+/- 0.27), -0.13 (+/- 0.37) and -0.11 (+/- 0.28) mm for PowerRef II. Regarding agreement with Neuroptics, the mean difference for low, mid and high mesopic conditions were, respectively: -0.48 (+/- 0.35), -0.83 (+/- 0.52) and -0.38 (+/- 0.56) mm for WAM-5500, -0.28 (+/- 0.56),-0.70 (+/- 0.55) and -0.61 (+/- 0.54) mm for PowerRef II. The mean difference of binocular minus monocular pupil measurements was: -0.83 (+/- 0.87) mm; and with proximity cues minus without proximity cues was: -0.30 (+/- 0.77) mm.
CONCLUSION: All the instruments show similar repeatability. In all illumination conditions, agreement of Neuroptics with WAM-5500 and PowerRef II is not good enough, which can be partially induced due to their open field design.Postprint (published version

In this study a commercial Hartmann-Shack aberrometer (KR-1W, Topcon Corp., Japan) and an Adaptive Optics Vision Analyzer based on a Hartmann-Shack aberrometer (AOVA, Voptica S.L., Spain) have been compared. In order to carry out this study, the wavefront aberration function expressed as a Zernike Polynomials expansion up to the fifth order of 29 right eyes has been obtained. A statistical analysis has been carried out by comparing the individual Zernike coefficients and the radial Root Mean Square (RMSrad) from the second to the fifth order, the high order RMS (RMSHOA) and the total RMS (RMSTOT). The values of the Sphere (S) and Cylinder (C) in Diopters (D) have also been compared. A paired sample t-test, Pearson correlations as well as the Bland and Altman method have been used to assess the agreement between devices.
